Cardiac Care: We provide individualized cardiac care plans to residents recovering from a heart attack or bypass surgery, or who may be experiencing abnormal heart rhythms or high cholesterol. We also offer individualized chronic cardiac care plans to residents with congestive heart failure or heart disease. These care plans provide for nutritional counseling, smoking cessation support, psychological interventions, exercise training, physical activity counseling and management of lipid levels, hypertension, weight and diabetes.

Dialysis Care: We offer dialysis support to meet the various needs of dialysis residents whether they are facing temporary or permanent loss of kidney function. We provide dialysis residents and their families with education about kidney disease, associated risk factors and recommendations for lifestyle modifications such as nutritional planning.

Diabetic Care: We provide diabetic residents with care plans and education designed to manage the disease and allow the residents to keep control of their lives. We monitor blood-glucose levels of diabetic residents and provides them with specific diet plans that won't aggravate their diabetes. Diabetic residents' feet, gums, and eyes are also closely monitored for side-effects from the disease and diabetic residents are taught improved methods for caring for their feet, gums and eyes to prevent and recognize signs of diabetic side-effects.

Dementia and Alzheimer's Care: If you have a loved one with dementia or Alzheimer’s, you understand the challenges of normal daily activities. Symptoms of Alzheimer’s disease may be further complicated by injuries, illness or end-of-life needs. We provide a nurturing and therapeutic environment where those with dementia or Alzheimer’s can receive the clinical and rehabilitation care they need in a safe and secure setting. We use the latest Alzheimer’s and dementia research to create individualized care plans and implement quality of life therapies such as healing touch, music therapy, and dementia-specific daily activities.

Discharge Planning: When patients are ready to return home, the goal of discharge planning is to help them do so safely. Discharge planning is a collaborative process between the care team and the patient that begins on admission. It involves assisting the patient and his or her family with acquiring the resources needed to be home safely such as medical care, therapy, home health, equipment and supplies, or making contact with community agencies that can provide any additional support, as well as a thorough evaluation of the home environment for safety concerns.

End of Life and/or Hospice Care: In the final stages of many terminal illnesses, care priorities tend to shift. Instead of ongoing curative measures, the focus often changes to palliative care for the relief of pain, symptoms, and emotional stress. Ensuring a loved one's final months, weeks, or days are as good as they can be requires more than just a series of care choices. We have more than fifty years of experience in end-of-life care and our compassionate staff will ensure that your loved one's last days are spent in a comfortable setting with warm, loving, and attentive staff surrounding them. We also partner with several hospice providers so that hospice care can be provided in the long term care facility.

Pain Management: We have extensive experience in managing residents' chronic pain problems. Our goal is to alleviate or reduce pain through the use of medical care. When patients have relief from pain, they are better able to enjoy their daily lives.

Pharmacy Services: We use Pharmacy Care Associates as our primary pharmacy services provider. Our alliance with Pharmacy Care Associates ensures that you receive your medications when you need them, right where you need them.

Respite Care for Hospice Patients: Providing care for terminally ill family members and friends is exhausting, both emotionally and physically. There may come a time during the course of your family member’s illness that you as a caregiver need some time away from the tasks of care giving. Caregiver stress can lead to physical and emotional distress that can interfere with your ability to provide care. We partner with many hospice organizations to provide respite care stays. Respite care programs are designed to provide temporary, short-term (usually, five days or less) assistance in caring for hospice patients. It is intended to allow the caregivers, often the family members, to take some time away from the hospice patient, to allow them to emotionally recharge and become better prepared to handle the normal day-to-day challenges of caring for someone with a chronic life-threatening illness.

Restorative Care: Our restorative care team evaluates every resident individually and develops care plans to reflect the resident's goals and to improve their wellness and function. The goal is to maintain optimal physical, mental and psychosocial functioning. Our restorative team works with each resident to identify activities that negatively affect his or her ability to engage in self-care or increase risk of injury. Our restorative team also assesses range of motion, getting in and out of bed, communicating, and ability to feed or dress oneself. We work with an interdisciplinary team to assess issues and nursing care that can enhance the resident's psychological adaptation to a decrease in function, increase levels of performance in daily living activities, and prevent complications associated with inactivity.

Stroke Care: The effects of a stroke vary depending on its severity - patients may experience difficulty walking, swallowing, communicating and performing daily activities. Residents with history of strokes are given individualized stroke care plans that address their unique stroke effects with the goal of regaining as much independence as possible. Stroke patients are also placed on various physical, occupational, and speech therapy regimens or a restorative program to help them regain function and physical coordination.

Wound Management: Residents recovering from surgery or extended illnesses may experience complex wounds that need further attention in order to heal properly. Our comprehensive wound care program is directed by a team of professionals who use the latest in wound management techniques and therapies help the resident heal faster. The wound care team works closely with other members of the health care team to prepare a plan of treatment that best addresses individual needs, and we involve the resident and the family in every step of the treatment plan.
